Jacob's Heart exists to improve the quality of life for children with cancer and other life-threatening conditions and support their families in the challenges they face. Our model of family-centered care includes emotional, psychosocial, practical and financial support to families living in Santa Cruz, Monterey, Santa Clara and San Benito Counties in California.
